Carefully fill out the form in the PDF below, then fax or mail the completed form to our office. You must include a copy of your signed court-issued fee waiver approval. Please do not send originals.

Once these documents have been received and approved, an email will be sent to you with your login code. This should happen within four business days after the receipt of your documents. This code will give you 30 days of access to the online program.
